When to Prolong Exhalation vs Inspiration

Use phase emphasis to target the predominant gas-exchange problem. Strongest evidence exists in obstructive disease for pursed-lip breathing (exhalation)1,2 and in hypoxemic states for inspiratory techniques3,4; durable ABG normalization is less consistent than acute physiologic benefit.

↑ Inspiratory time → alveolar recruitment, better ventilation distribution. Hypoxemia (↓PaO₂) from atelectasis, V/Q mismatch, shallow breathing patterns. ↑ PaO₂ (modest); PaCO₂ effect variable3,4. No consistent durable ABG shifts; long-term benefits more functional4,5,6.

Quick Use Tips

  • If ↑PaCO₂ is the issue: coach longer exhalation (PLB) during exertion and recovery1,2.
  • If ↓PaO₂ is the issue: coach slower, fuller inspiration with a brief inspiratory hold and controlled release3,4.
  • Reassess ABG or SpO₂ trends within minutes; track for expected direction of change and patient comfort.
